Due to Covid-19 restrictions, all in-person Medicare and Your Choices Workshops presented by the ADRC Elder Benefit Specialist have been temporarily suspended. If you wish, you may have a Medicare packet mailed to you.  Fill out and submit the form below, call the ADRC at (262) 335-4497, or request via Email.  The attached video presentation will guide you through the materials and provide information on the following subjects:
