Aging and Disability Resource Center
The Aging & Disability Resource Center (ADRC) is a place where information, assistance, and access to services and community resources is in one central place for seniors and adults with physical and developmental disabilities. Adults with mental health and/or substance abuse issues can be connected to assistance through other resources, including the ADRC. Family members, friends, neighbors, caregivers, doctors, and other professionals can also receive information through the ADRC to assist them in their work with seniors and disabled adults. This assistance can be provided at the ADRC, over the phone, or during a visit to an individual's home. These services are available regardless of income or health status with no cost to the individual.
To report suspected abuse (physical abuse, sexual abuse, material abuse) or neglect and/or self-neglect of persons 60 and over or any adult with a physical, cognitive or developmental disability, contact the Aging & Disability Resource Center to speak to the Adult Protective Services (APS) Social Worker. APS will investigate the report and offer services to ensure the safety and well being of the subject. The person does have the right to refuse services unless (1) the vulnerable person lacks sufficient mental capacity to protect him or herself. In those cases a protective placement may be initiated or (2) a crime has been committed and law enforcement takes action.

State of Wisconsin Board on Aging and Long Term Care - Ombudsman: Responds to compaints and problems of residents or recipients of long term care such as nursing homes, CBRF's, and group homes. The purpose of the Benefit Specialist program is to protect the interests of adults age 60 or older and adults with disabilities age 18 and older in a broad range of areas. Family Care Program
Family Care is a publicly funded long-term care program that has replaced the COP and CIP Waiver programs in Washington County. The program offers community based services to the elderly, and to physically and developmentally disabled adults who are in need of long term care such as would be provided in a nursing home setting and paid for by medical assistance (Title 19/MA or Medicaid). To be eligible for services under Family Care, a person must meet financial and functional requirements. Health and Wellness Screening
Nancy Gloede, R.N. provides health and wellness screening, which includes blood pressure screening and patient education. Patient education includes reviewing medications and their side effects, answering questions about medical conditions and explaining tests and treatments. It may also include clarifying what your doctor has told you about a medical condition. This service is free of charge and available to any resident of the county aged 60 and older.

There are two No-Call Registries available if you choose to receive fewer telemarketing calls at your home or mobile phone. If you contact the National Do Not Call Registry, your registration will be effective for 5 years and you can register your home or mobile phone for free. Telemarketing calls should lessen once your phone number has been on the registry for 31 days. You can either call 1-888-382-1222 or register on-line.
Consumers can sign up on the Wisconsin No Call List 24 hours a day, 7 days a week, 365 days per year. The updated no call list is given to registered telemarketers quarterly so it may take 30-120 days for unwanted calls to lessen. Registration is effective for 2 years. Alzheimers Services
Alzheimer's Family and Caregiver Support Program (AFCSP) A state funded program adminstered by the Wisconsin Department of Health Services and operated by county boards and local agencies. The program makes an array of community services available to prevent or delay institutionalization of people with Alzheimer's Disease and to assist family members and others who provide unpaid care. To be eligible for the program there must be an Alzheimer's Disease diagnosis, or other irreversible dementia, and the individual must meet income/asset requirements.

Any Wisconsin Senior may be eligible for SeniorCare if you are a permanent resident of Wisconsin, and age 65 or older. There are three levels of program participation, which are dependent on your income. Only your gross income and/or gross social security is considered. Your assets - home, car and other things you own - are not counted.
If your prescription drugs are covered under another health insurance plan, you can still be enrolled in SeniorCare. SeniorCare will coordinate coverage with your other plan.
You will pay $5.00 per prescription for generic drugs and $15.00 per prescription for brand name drugs. Over the counter drugs, such as vitamins and aspirin are not covered, even if prescribed.
There is an enrollment fee of $30.00 and it takes approximately 3-4 weeks for your paperwork to be processed. You will then receive a SeniorCare card to be used each time you need to fill a prescription.
The following is a schedule of income levels:
LEVEL ONE:

SeniorCare and has also been deemed "creditable coverage" (equivalent or better) with regards to Medicare Part D, so the penalty associated with Medicare Part D will not be accumulate while enrolled in SeniorCare. The SeniorCare program is scheduled to end in December 2012.
Medicare Part D Prescription Drug Benefit
Beginning January 1, 2006, people with Medicare had the option to enroll in a plan that covers prescription drugs. This new benefit is known as "Part D."
Who Is Eligible?
Anyone with Medicare Part A (Hospital Insurance) and/or Part B (Medical Insurance) can join a Medicare prescription drug plan. A person must live in the service area of a prescription drug plan to enroll.
When Can I Join?
If you currently have Medicare Part A and/or Part B, you can join a Medicare prescription drug plan between November 15 and December 31. Your Medicare prescription drug plan coverage will begin on January 1. Part D is a voluntary benefit. This means that an individual who receives Medicare benefits may choose to decline Part D coverage. However, if you delayed joining a Medicare prescription drug plan after you were first eligible, you are likely to pay a penalty in the form of a higher monthly premium. You will have to pay this higher premium as long as you have a Medicare prescription drug plan. The penalty will not apply if you currently have a drug plan that covers at least as much as a Medicare prescription drug plan.
Who Will Provide Medicare Beneficiaries With Drug Coverage?
Insurance providers and other private companies will offer Medicare Part D plans approved by the federal government.
What Drugs Will Be Covered By Medicare?
All plans must include coverage of certain drugs commonly prescribed to elderly and disabled people. Each Medicare prescription drug plan will develop a list of brand name and generic drugs it will cover, called a formulary. The government has not established a uniform list of drugs that every plan must cover. Therefore, consumers will have to compare the formularies offered by each plan available in their region, and enroll directly with the plan of their choice.
How Much Does It Cost?
Costs under Part D will vary according to a Medicare beneficiary's plan choice, medication needs, and income level. Many Medicare beneficiaries will have to pay a monthly premium for Part D coverage as well as a yearly deductible (up to $310 for 2010) and other out-of-pocket costs. Each drug plan can create its own cost structure - within a certain framework created by the federal government. Some plans may offer more coverage and additonal drugs for a higher monthly premium. Medicare beneficiaries with lower incomes and few assets, as well as Medicare beneficiaries who also have Medicaid coverage, will receive assistance with out-of-pocket costs associated with Part D.
What Happens If I Have Other Prescription Drug Coverage?
Many Medicare beneficiaries already have prescription drug coverage through another source such as an employer, union, Wisconsin SeniorCare, Medicaid and Medicare supplemental (Medigap) insurance. If you have been notified that your coverage is creditable, you can continue with your coverage without accruing any penalty with Part D. Medicare beneficiaries who currently receive prescription drug coverage under the Medicaid program will no longer have their drugs paid for by Medicaid after December 31, 2005. People with Medicare who also get full Medicaid benefits will need to enroll in a Part D plan of their choice or will be automatically enrolled in a Medicare prescription drug plan by the government. They will have the opportunity to switch to a different plan on a monthly basis if they need to.
Important Dates:
The Annual Enrollment Period is November 15 through December 31 every year, with your coverage beginning on January 1. Late penalties of approximately 1% per month will apply if you delayed enrollment and did not have creditable drug coverage.

The congregate nutrition program offers persons age 60 and over the opportunity to enjoy a hot lunch and to meet socially with other seniors. There are seven congregate meal sites located throughout Washington County that offer noon meals up to five days per week.
Reservations for lunch must be made one day in advance by 12:00 p.m. by calling 262-335-4497 or by calling the meal site directly. Cancellations should also be called in by no later than 12:00 pm one day in advance. The seven meal sites are:
