Alzheimers Services
Alzheimer's Family and Caregiver Support Program (AFCSP) A state funded program adminstered by the Wisconsin Department of Health Services and operated by county boards and local agencies. The program makes an array of community services available to prevent or delay institutionalization of people with Alzheimer's Disease and to assist family members and others who provide unpaid care. To be eligible for the program there must be an Alzheimer's Disease diagnosis, or other irreversible dementia, and the individual must meet income/asset requirements.

Adult Day Care The following facilities have programs that provide supervised care for dependent adults in licensed group facilities during some portion of a twenty-four hour day. Services may include social and recreational activities, training that is essential for sustaining the activities of daily living and meals, as appropriate.
